Primary responsibilities will be to assess the patients presenting needs and work to develop a plan of care that may include brief therapy, connection to resources and follow up care..Job Description:KEY ACCOUNTABILITIES:1. Assesses, diagnoses when necessary, and evaluates patient's status and the effectiveness of mental health care using tools deemed appropriate by clinical setting, such as the Diagnostic and Statistical Manual of Mental Disorders (DSM-5), to create an individualized plan of care that includes cultural diversity and special needs appropriate to the patient population served. 2. Continually assesses patient's progress and provides interventions and strategies to support the mental health diagnosis. 3. Prepares and maintains all required treatment records, reports, and other clinical information on time and accurately using current technology in accordance with regulatory requirements and department procedures. 4. Works collaboratively with the patient and others to develop a comprehensive treatment plan that addresses the needs of the patient, desired outcomes, and agreed upon plan of care. 5. Develops treatment plans and objectives in collaboration with patient and family, as appropriate, based on patient needs/goals. Anticipates and intervenes in areas of patient safety and symptom reduction. Is able to work in a team environment, remaining attentive to ideas, recognizing responsibilities, and actively participating with others to accomplish assignments and achieve desired goals. 6. Coordinates referral options and serves as a link between patients and treatment providers. Establishes and maintains effective working relationships with other clinicians or other referral agencies 7. Provides education and mentoring for students, interns, residents, observers and volunteers. 8. With direction from your clinical supervisor, you may be asked to provide peer-to-peer review, clinical collaboration, chart auditing, coaching and mentoring in an effort to support team member development.QUALIFICATIONS:PHYSICAL DEMANDS:QUALIFICATIONS: Master's degree in Social Work and current State of Wisconsin License as Licensed Clinical Social Worker (LCSW). Experience with Substance Use Disorder Counseling and State of Wisconsin licensure as a SAC or CSAC preferred. Must be comfortable working with patients who are experiencing a mental health or substance use related crisis.Ability to move freely (standing, stooping, walking, bending, pushing, and pulling) and lift up to a maximum of twenty-five (25) pounds without assistanceJob classification is not exposed to blood borne pathogens (blood or bodily fluids) while performing job dutiesWORK ENVIRONMENT:Climate controlled office setting with daily movement throughout the facilityInteraction with department members and other healthcare providersPosition requires compliance with department specific competencies.I understand that this is a summary of job responsibilities and does not create contractual terms and conditions and is subject to change in order to meet organizational needs.
