Job Description:

Responsible for obtaining client information, baseline data, consent and completing appropriate forms. A primary liaison between Piedmont Healthcare, the community, and network partners to ensure appropriate linkage of services and referrals. Coordinates program activities in preventing disease, promoting and protecting health and minimizing complications of health problems in target population. Knowledge of community resources, treatment modalities, and the present health care delivery system, both hospital and community based.

Education

Bachelors Degree is required.

Experience

Five years experience in community health, case management acute care, or LPN licensure with administrative experience is preferred.

Licensure/Certification

None.

Skills

In-depth knowledge of medical terminology/disease process and available community resources, ability to comprehend complex rules and regulations of various accrediting and licensing agencies, ability to obtain basic vital signs and lab collection or willingness to obtain training.

Above average communication skills and the ability to communicate effectively to the public and health care professionals.
