This Home Health Physical Therapist opportunity is based in the
Greenville, Alabama area, a welcoming small city known for its

collaborative, interdisciplinary team dedicated to helping patients
remain safe and independent in their own homes. As a Home Health
Physical Therapist, you will have the opportunity to form
meaningful, one-on-one relationships with patients and families,
see the direct impact of your care on daily life, and practice with
a high level of professional autonomy.Typical responsibilities
include performing comprehensive physical therapy evaluations in
the home, developing and updating individualized plans of care, and
coordinating with physicians, nurses, occupational therapists,
speech therapists, social workers, and home health aides. You will
provide skilled interventions such as therapeutic exercise, gait
and balance training, transfer training, home safety and fall
prevention strategies, and education for patients and caregivers on
equipment use and energy conservation. You will also conduct home
safety assessments and recommend modifications or assistive devices
to enhance independence and reduce risk of injury.A typical day in
this role includes traveling between patient homes in Greenville
and surrounding communities, managing a schedule of home visits,
performing assessments and treatments in varied home settings, and
completing timely electronic documentation. Caseloads generally
focus on adult and geriatric clients with orthopedic, neurologic,
cardiopulmonary, and medically complex conditions, including those
recently discharged from the hospital or skilled nursing
facilities. You will play a key role in helping patients transition
safely from higher levels of care, regain function, and remain in
their preferred home environment.The work environment is supportive
and team-oriented, with an emphasis on communication and
collaboration even though much of the care is delivered
independently in the field. You can expect regular case
conferences, ongoing clinical support, and opportunities for
professional growth, including further specialization in areas such
as balance and fall prevention, chronic disease management, or
post-surgical rehabilitation. This home health setting is
