Job Description:
Encompass Health Corporation is seeking a dedicated Case Manager
II to join our interdisciplinary team of professionals in the
Social Services department. This role is critical in facilitating
patient-centered care throughout our inpatient rehabilitation
hospitals and home health services. The successful candidate will
be responsible for assessing patient needs, planning and
coordinating health services, and ensuring a seamless transition
between various levels of care. The goal is to enhance the quality
of patient management and satisfaction, maximize continuity of care
and optimize health outcomes. Conduct comprehensive patient
assessments to identify health needs and eligibility for specific
services. Develop individualized care plans in collaboration with
healthcare professionals and communicate with patients and their
families about care options. Advocate for patient needs as part of
a multidisciplinary team to ensure that appropriate medical
services are provided. Organize referrals to other medical
specialists, arrange for medical transportation and follow-up
appointments, and verify patient adherence to medical advice.
Monitor and evaluate the effectiveness of the care plan and make
necessary adjustments. Document and maintain accurate records of
patient cases and outcomes. Stay informed on the latest healthcare
practices and regulations. Strong interpersonal and communication
skills Ability to work collaboratively in a multidisciplinary team
Proficiency in healthcare management systems and electronic
healthcare records Expertise in developing and administering care
plans Strong organizational and time management skills Medical
Insurance Dental Insurance Vision Insurance 401k Certified Case
Manager (CCM) preferred Valid nursing or social work license in the
state of practice
