About Us Our organization has pioneered the first national in-home
clinical network, establishing itself as an independent provider of
thorough health assessments for individuals nationwide. We are
dedicated to delivering outstanding, personalized care that
enhances health, reduces costs, and empowers individuals to lead
fulfilling lives. With a strong foundation in clinical assessment
and care management services, our extensive network of clinicians
transcends traditional barriers by meeting members in their own
environments. We aim to help older adults and other at-risk
populations achieve better care quality, improved health outcomes,
and identify chronic conditions that may otherwise remain
undetected. Joining our team allows you to utilize your skills and
compassion to make a meaningful difference in the health and
well-being of others. Be part of our mission and enjoy competitive
base pay along with incentive compensation and benefits while
positively impacting the community! Responsibilities About the Role
During visits that may last up to one hour, our providers assess
and observe a member’s current health status, medical history,
medication adherence, social circumstances, and other potential
risks. This approach provides invaluable insights into a member’s
overall health that are often missed during standard office visits.
The Matrix Comprehensive Health Assessment (CHA) enhances care
quality and allows us to address multiple care gaps in a single
appointment. Our providers are responsible for: Conducting
assessments for adults and geriatrics, including medical history,
diagnosis and treatment, health education, referrals to physicians
and case management, follow-up, and thorough documentation in
accordance with our guidelines. Administering and ordering
necessary point-of-care and lab-based tests. Collaborating with
physicians, case managers, social workers, family members, key
caregivers, and other medical personnel as appropriate. Working
alongside Primary Care Physicians (PCPs) to provide patient
education and follow-up as needed. Delivering services at the
member’s residence, which may include their homes or Skilled
Nursing Facilities. Benefits of Working with Us Consistency – While
assessments are conducted from Sunday to Saturday between 8 am and
7 pm, our full-time providers generally work Monday to Friday from
8:00 am to 5:30 pm. Competitive Pay – Enjoy an annual base salary
ranging from $95,500 - $158,400 , determined by market analysis and
experience. Incentive Program – Participate in monthly (uncapped)
and year-end incentive opportunities. Comprehensive Benefits –

onboarding training, you will be paired with a preceptor, receive
personalized feedback, and gain free access to online continuing
education. Additional Licensure – If business needs arise, we will
assist you in obtaining additional state licensure and

while conducting health assessments. Qualifications Skills &
