Position Overview: This role is designed for a part-time geriatric
physician who will deliver outpatient services to elderly patients
and those on Medicare. The focus will be on evaluating, treating,
and providing care recommendations for prevalent age-related health
issues. The physician will work alongside a multidisciplinary team,
which includes nurses, pharmacists, and social workers, to enhance
patient health and overall quality of life. This position involves
collaborating with the Elder Care team to support members aged 65
and older, as well as those with Medicare who require specialized
care. The primary emphasis is on outpatient services, including
assessments, treatments, and recommendations for conditions
frequently encountered in older adults, such as memory impairment
and falls. The physician will closely partner with a Geriatric RN
Care Coordinator, a Clinical Pharmacist specializing in Geriatrics
and Palliative Care, and a Geriatric Social Worker. Patient
interactions will primarily take place in medical offices, either
in-person, virtually, or via phone. In conjunction with the Primary
Care Physician (PCP), the physician will address medical issues and
chronic conditions, focusing on enhancing patients' quality of life
and functional capabilities, which includes home safety evaluations
and discussions on end-of-life care when appropriate. This is a
part-time role, requiring 20 hours per week, based at our Sandy
Springs Medical Office. Key Responsibilities: Conduct telephone,
virtual, and in-office consultations. Evaluate, treat, and/or
recommend care for patients referred to the Geriatrics department.
Engage in discussions regarding care goals and document advance
directives as necessary. Address clinical concerns and formulate
the medical components of care plans, including recommendations for
observations, lab tests, medications, referrals, and prescriptions,
to be shared with the patient's PCP and other care team members to
optimize health outcomes. Respond promptly to MD advice and
referrals, providing appropriate clinical feedback to referring
providers. Timely address e-consults from the PEEC clinic with
suitable recommendations for patients' pre-surgical needs.
Collaborate with specialists in Palliative Care, Neurology,
Neuropsychology, Behavioral Health, and Clinical Pharmacy to ensure
comprehensive patient care when necessary. Supervise Geriatric
Nurse Practitioners, review and approve notes, provide feedback on
care plans, and authorize prescribed medications and other orders.
Educate Primary Care and other specialists about geriatric patient
care as requested. Minimum Education and Experience: MD/DO with
completed residency training and board certification in Internal
Medicine or Family Medicine. At least one year of post-residency
experience in treating older patients in outpatient or facility
settings. Proven ability to communicate effectively with elderly
patients, their families/caregivers, and non-physician healthcare
professionals. A passion for providing exceptional care and
experiences for older members. Willingness to travel to various
locations for patient care. Preferred Education and Experience:
Completion of a fellowship in Geriatric Medicine. Subspecialty
board certification in Geriatric Medicine (for Internal Medicine
physicians) or a Certificate of Added Qualifications (CAQ) in
Geriatric Medicine (for Family Medicine physicians). Two years of
post-residency experience in caring for older patients in
outpatient or facility settings. Experience working within an
