Description: Summary: Under general supervision, assesses,
screens and counsels consumers in a variety of program areas to
enhance, meet or restore a patient's/client's/consumer's
functioning capacity. Develops or participates in development of
care plans. Provides case management/case coordination services to
consumers Provides counseling/therapy to consumers requiring
complex intervention, including those who are homicidal, suicidal
or exhibit life-threatening behaviors; determines necessity for
involuntary commitment; may execute orders of involuntary
commitment; provides clinical training to professional social
services staff; performs assessments for court depositions, legal
proceedings and eligibility determinations. May supervise
para-professional level staff. Licensure as a Licensed Clinical
Social Worker, Licensed Professional Counselor or Licensed Marriage
and Family Therapist is required. Duties and Responsibilities: A.
Assembles information, screens referrals and applicants for
eligibility, and assigns service priority. (Performed by all
incumbents)1. Has knowledge of the clinical/service needs of the
following population(s): Adult MH MR/DD SA Youth MH MR/DD SA 2.
Screens referrals and applicants within an established period based
upon level of need (emergent, urgent, routine) to determine
eligibility and appropriateness for services.3. Accurately
assembles medical, psychosocial and financial information necessary
to process admissions/referrals.4. Assigns appropriate priority to
eligible applicants and/or refers ineligible applicants to other
community resources in accordance with established procedures.5.
Maintains adequate knowledge of disability/age specific community
resources based on knowledge of program criteria.B. Conducts
advanced level psychosocial assessments/evaluations; utilizes
information from other service providers, physicians, team members
and consumer/representative to develop comprehensive service plans
and use in court depositions, etc. (Performed by all incumbents)1.

factors that place the consumer at risk for suicide or self-harm,
violence, medical disorders, substance abuse Functional
strengths/deficits, including sources of motivation, resources,
capabilities Consumer knowledge/education needs, including
understanding of his/her illness, medication, and treatment and
obstacles to learning Medical problems, especially with regard to
how they are causing or contributing to behavioral health issues
Critical stresses that negatively affect the consumer's status,
including financial, legal, social factors Cultural factors
including how culture affects management of stress, family roles,
and other natural support Consumer preferences with regard to
services, including preferences regarding family involvement
History of mental health disorder(s) as to age of onset, duration,
patterns and consequences to both consumer and family Spirituality,
including consumer's feelings of hope about the future and ability
to identify sources of strength and motivation Types of previous
treatment and responses to that treatment4. Utilizes program
specific format to develop and document individualized service plan
in a consistent and accurate manner.5. Completes and reviews
service plan within program specific time frame.6. Appropriately
utilizes assessment information for court depositions, legal
proceedings, and in making consumer eligibility determinations on
Social Security/Medicaid matters.C. Conducts periodic
reassessments/ISP reviews to determine continued eligibility and/or

Completes required consumer reviews within prescribed time frames
as established by utilization guidelines and submits, where
applicable, TRIGRS to the Utilization Management Department for
authorization or reauthorization of services.2. Documents review
findings in the consumer's medical record, and updates the ISP to
reflect revised goals and new target dates.3. Effectively secures
additional and/or continuing services for consumer in accordance
with his/her needs.4. Communicates changes in consumer's status and
plan of care to members of the treatment team.D. Provides services
to individuals, groups and families requiring complex professional
intervention using a variety of psychotherapeutic techniques.

and group counseling techniques most appropriate to the above noted
population(s), including the following:Non-directive techniques,
Cognitive techniques, Behavior modification techniques, Modeling,
Conflict resolution, Negotiation, Closure techniques2. Demonstrates
ability to deal productively with the following consumer types
within an individual and group context:Withdrawn, Disruptive,
Depressed/suicidal, Manipulative, Psychotic, Manic3. Evaluates the
suitability of consumers for therapeutic intervention in accordance
with accepted standards of clinical practice.4. Establishes
realistic goals and time frames for services with consumers,
families, and other members of the treatment team.5. Ensures
documentation reflecting continuity of care is completed in a
timely and efficient manner, including progress notes which are
consistent with the ISP and authorized services and reflect
progress toward achievement of service goals.6. Analyzes results of
services and indications of progress and altars the therapeutic
approach accordingly.7. Designs behavioral or therapeutic
interventions for the more difficult to manage consumers.E.
Establishes linkage and coordination of services with community
agencies, both public and private. Develops and maintains a
cooperative working relationship with community service providers.

welfare programs for which they may qualify within program specific

involved with the care of the consumer through individual contact
and attendance at treatment team meetings.5. Adheres to all ethical
and legal standards when interacting with courts, the general
public, and other agencies.6. Provides requested information
regarding agency services to community service providers in a
timely manner and in accordance with agency policy.7. Meets with
consumer and family to develop community support plan in order to
maintain level of functioning upon discharge.F. Serves as clinical
preceptor for students, interns and candidates for licensure or

ensure that trainees receive thorough and well-rounded clinical
experience.2. Considers difficulty of caseload and capabilities of

authority.4. Intervenes in cases assigned to trainees as necessary

Qualifications: Licensure as a Licensed Clinical Social Worker,
Licensed Professional Counselor or Licensed Marriage and Family
Therapist by the Georgia Composite Board of Professional
Counselors, Social Workers, and Marriage and Family
